Now here you need to select "Google Search" option and process to next step.

After clicking on google search now here you need to select Intellectual Property Issue as shown in below image.

Here you need to select Copyright infringement and then you need to declare that whether you are the Copyright owner or authorized to act on behalf of some one.

And select "Others" option in allegedly infringing work in question.

Now After Clicking on create request you will be redirected to a new page and now you can fill the information like Name, company name or Blog Name, Country, etc.

Here in the first box you need to provide brief information regarding the incident and in the second box you need to paste your blog or website URL and in third box you need to paste the blog or website URL which you want to remove from search results.

Now here you need to accept Terms and Conditions and then sign the document and submit it for review.

Now you have successfully submitted DMCA takedown request for review, and you can check the status by visiting Google DMCA Dashboard webpage.
